I care for patients brought to me unconscious several times a day. In a truly free-market health care system, they might have died.

Elective treatments may remain elective and free market.
Emergency medicine is not elective.  It should be paid for by
the common funds.  There are conditions that are not elective and not emergencies.  they should be paid for out of the same funds. They can wait on the convenience of the medical establishment.  Cash can shorten the wait.
