The report said that neglect reports filed in 2012 and 2013 about Bella’s mother, Rachelle Bond, should have received a higher-level response, and that those cases were closed prematurely. The report also said caseworkers failed to gather current information on Ms. Bond and did not adequately assess her parenting ability. The state had previously taken two other children of Ms. Bond’s children from her care because of her long history of arrests and drug use. Ms. Bond’s boyfriend, Michael P. McCarthy, has pleaded not guilty to killing the toddler, and Ms. Bond pleaded not guilty to covering up her death."
