Underpaid and under supervised technicians get sloppy with even the most pathogenic and toxic materials.  They do not respect the vagaries of fashion. Often they know what they are doing.  As often they do not know.
Sometimes they are drunk or stoned.
They are doing as they have done before and trying to self supervise.
Give them a checklist and let them get on with the job.
The scientists are doing science.
The soldiers are soldiering.
Hire and pay process engineers.
